Date Coverage Is Effective
Coverage under the plan shall become effective with respect to an eligible employee and eligible dependents on the first day of the month following the first day of full-time employment.*

Eligible Dependents
The MPS benefit-eligible employee's spouse and children as defined in the "Eligible Classes of Dependents" section of the MPS Insurance Administrative Guidelines are eligible for medical, dental and vision insurance coverage.

Dependent Care Reimbursement Account (DCRA)
This account allows employees to set aside a portion of income before it is taxed to pay for dependent care expenses. Please note that funds do not roll year to year. Any unused funds will be lost.

Changes to Benefits Elections
Employees may make changes to their insurance during Open Enrollment or upon experiencing a qualifying life event (i.e. birth/adoption, marriage, divorce, loss of insurance coverage, death or other specific circumstances). Changes must be made within 30 days of the date of the event. The employee must notify the Manager of Benefits at 989.923.5067 or benefits@midlandps.org upon experiencing the event and to request any changes. The Manager of Benefits will notify the appropriate insurance companies.

Medical Insurance With Prescription Drug Coverage
Medical insurance is provided through MESSA/BCBSM (Michigan Education Special Services Association/Blue Cross Blue Shield of Michigan). Eligible MPS employees are enrolled in MESSA's ABC Plan 1 which is a High Deductible Health Plan (HDHP) with a Health Savings Account (HSA) and in MESSA's ABC Rx Prescription Plan. MESSA’s Member Services number is 800-336-0013 or you can log on to their website at messa.org. MPS employees pay a premium for their medical insurance calculated by a coverage-level percentage of their base annual wages.
Health Savings Account (HSA)
MPS benefit-eligible employees with MESSA's medical insurance coverage may enroll for a Health Savings Account administered by HealthEquity. HealthEquity's Member Services may be reached at 866-346-5800. You can create an account at myhealthequity.com to view your fund balance and more.
Midland Public Schools contributes funds annually to each employee's HSA to help cover part of the deductible.
- During 2025, MPS will contribute a total of $1,325 to an employee's HSA with single medical coverage or $2,625 to an employee's HSA with two-person or family coverage.*

Employees may contribute additional money into their HSA pre-tax from their paychecks. There are IRS guidelines each year that set the maximum amount allowed to be contributed into an HSA (employer and employee contributions combined).
Dental Insurance
Dental insurance is provided through Cigna Healthcare. Cigna's Member Services may be reached at 800-244-6224. You can create an account to see details of your dental coverage and use at mycigna.com. MPS benefit-eligible employees do not pay a premium for their dental insurance. It is a benefit provided by Midland Public Schools.
Vision Insurance
Vision Insurance is provided through MESSA/VSP-3 Plus (Vision Services Plan). The Member Services number for VSP is 800-877-7195. You can create an account to see details of your vision coverage and use atvsp.com. Each MPS benefit-eligible employee is automatically enrolled for vision insurance even if vision insurance is waived. MPS benefit-eligible employees do not pay a premium for their single vision insurance. It is a benefit provided by Midland Public Schools. There is an option to add dependents to your vision insurance at a cost to the employee.
Life/AD&D Insurance
Life/AD&D (Accidental Death & Dismemberment) insurance is provided through Reliance Standard Life Insurance Co. Eligible employees receive a life insurance benefit of two times their salary, rounded up to the next thousand. AD&D has a schedule of benefits for loss of life or limbs due to an accident, up to the life benefit. Beneficiary change forms are available on request from the Manager of Benefits. MPS benefit-eligible employees do not pay a premium for their Life/AD&D insurance. It is a benefit provided by Midland Public Schools.

Schools' employee retirement plans are handled by the Office of Retirement Services in Lansing, MI. New MPS employees will receive an information packet from the ORS about two weeks after their first paycheck and will need to make a retirement election within 75 days of their first paycheck. Log on to miAccount with the Member ID you receive from the Office of Retirement Services (ORS) to view your retirement election and years of service. There is a retirement calculator as well as the form to apply for retirement available at this website. If you have lost or forgotten your Member ID or password you can request the information at the website. The ORS employee services number is 800-381-5111 or you can use the miAccount Message Board.
If you are enrolled in the Pension Plus, Pension Plus 2, or the Defined Contribution retirement plan you will have an account with Voya to manage your 401(k) and/or 457 accounts. You can review and make changes to your account by logging in to stateofmi.voya.com. There are educational programs, such as webinars and 1 on 1 appointments, available at this link: www.stateofmiplanseducation.com.

Fundraising
Before beginning a fundraiser, fill out the approval form linked below including all details requested. Approval from a building administrator prior to submitting this form is required (and is to be documented in the appropriate section of the form.) You will receive a notice of approval or denial via. e-mail from the Associate Superintendent of Finance and Human Resources within one week of submission. An approval will be accompanied by an authorization number that will be required on the financial (profit/loss) report.
These forms apply only to school-sponsored groups that will be depositing funds into an MPS account. External affiliates such as PTO's and Booster Clubs that have a 501c3 status should follow their internally established guidelines.
Please note that governmental standards do not allow school districts to disburse funds to charitable organizations. If your student organization desires to organize a campaign with this intention, please talk to your school administrator for guidance.
These forms will become active and accept approval requests on December 1st, 2021 for fundraisers beginning in the 2nd semester of the 2021-2022 school year (January 17th, 2022).

Payroll FAQs
The Midland Public Schools Business Office staff is responsible for processing biweekly pay for approximately 970 school district employees. This is done in compliance with all applicable federal and state law, and current bargaining agreements.
Employee Access Center (EAC) – online site that houses your payroll, benefit and demographic information. Your network username and password, or your Employer ID number in the username field.

Online pay statements are available on scheduled pay dates. The online pay statement may be available before the actual pay date, but funds will not be posted to your account until the actual pay date.
- If your paycheck was not in EAC, contact the employee responsible for entering your attendance.
- If there is a check listed in EAC, contact the business office.
- If it is your first paycheck of the new fiscal year, please contact the payroll specialist at payroll@midlandps.org, or at the number below in the contact us section.
- If it is during the school year, and you notice an inaccuracy in your leave amounts, please contact your building Administrative Assistant.
Miscellaneous Payment Requests, Reimbursement Requests and Mileage requests go through additional approval processes. It may take one to two paychecks for you to see the extra payment on your paycheck.
All requests for payment must be approved and in the business office no later than 2:30 on the Friday before a payroll Friday.
